- Fentanyl Patch Return Program
Fentanyl misuse, overdose and deaths are an increasing problem in the Leeds, Grenville & Lanark region. The Patch 4 Patch initiative is designed to work with physicians, pharmacists and patients to promote and regulate the safe use of Fentanyl patches.
The Patch 4 Patch program works by having the physician and pharmacist dispense a certain number of Fentanyl patches at one time and have the patient return all of those patches to receive more form the pharmacy. This initiative will help to eliminate the illegal diversion of patches to the community and decrease illicit Fentanyl use.
Please find below the Fentanyl Patch 4 Patch documents for physicians and pharmacists, please contact Jennifer Adams at the Health Unit if you are planning to start a patch 4 patch program through your pharmacy or family health team for monitoring purposes.
If you believe that your fentanyl use is becoming a problem in your life please talk to your health care provider for help and more information. If you are concerned about having a fentanyl overdose please contact the Leeds, Grenville & Lanark District Health Unit for more information about receiving training for a Naloxone Kit, call 1-800-660-5853 or visit the Revive page
