Infection Prevention, and Control Guidelines for Primary Care Physicians

Reportable Diseases

Introduction:

Ontario's Health Protection and Promotion Act states that certain infectious diseases or suspected occurrences of these diseases must be reported to local health units by health care providers, laboratories and administrators of institutions. Health units in turn must report these diseases, known as reportable diseases, to the Ministry of Health and Long-Term Care. These diseases are identified on Ontario's reportable diseases list. Diseases that are deemed reportable require that health care professionals of the Family Health Team report any incidence of these diseases because it is a public health risk. The source of the disease needs to be investigated in order to prevent others from getting infected and transmitting the disease.  A list of Reportable Diseases is included.


Best Practices:

Reporting:

  • All reportable diseases must be reported to the Leeds Grenville and Lanark District Health Unit for investigation and follow-up

  • The reportable diseases marked are deemed to be of high urgency and should be called to the Health Unit immediately.  The Health Unit has 24-hour on-call system.

  • When a health care provider develops the opinion that a patient has or may have (e.g. query or suspect) a reportable disease, the provider must report to the Health Unit as soon as possible

  • Report using the Reportable Disease Notification Form (included). It is important that all details are completed. The name, address in full, date of birth, sex and date of onset of symptoms is the minimum amount of information mandated for reporting.

  • The Reportable Disease Notification Form should be completed by the professional providing care to the patient.

  • The Reportable Disease Notification Form contains confidential information and must kept as a record on the patients chart.

  • Each disease does require additional information. The Health Unit may contact the professional providing care to the patient after the report has been made.
