The following specified Reportable Disease, (Ontario Regulation 559/91 and
amendments under the
Health Protection and Promotion Act, 1990) is to be reported to
the Local Medical Office of Health.
Note: Diseases marked
and all respiratory infection outbreaks in institutions should be
reported
immediately to the Medical Officer of Health by telephone. Other diseases are to
be reported
the next working day.

Surveillance Case Definition: Lyme Disease
Confirmed Case: Endemic
a)isolation of Borrelia burgdorferi from a clinical specimen or
b)erythema migrans observed by a physician or
c)at least one clinically compatible late manifestation (neurologic,
cardiac, or musculoskeletal) and laboratory evidence of B. burgdorferi
infection
PLEASE NOTE THE 1000 ISLANDS REGION IS AN ENDEMIC AREA
THE HEALTH UNIT WILL ARRANGE FOR TESTING OF TICKS REMOVED FROM PATIENTS
FREE OF CHARGE.
Confirmed Case: Non-endemic
a) erythema migrans observed by a physician and laboratory evidence
of B. burgdorferi infection
Probable Case
Endemic:
a) physician recognition of erythema migrans as reported by patient
Non Endemic:
a) Compatible late manifestation(neurologic, cardiac or musculoskeletal)
and laboratory evidence for B. burgdorferi
