The Strategic Plan of the Leeds, Grenville and Lanark District Health Unit for 2013 to 2018

This Strategic Plan will provide direction to the work of the Health Unit for the years 2013-2018. The previous Health Unit Strategic Plan, Moving Upstream 2006-2012, recognized the importance of looking at the underlying factors that influence health and health behaviours and was grounded in a commitment to continuous quality improvement and the principles of organizational excellence.

Moving Upstream 2013–2018 builds on the foundation of the previous plan. The plan consists of 3 key components:

  1. Health Unit Identity, consisting of mission, vision and value
    statements.

  2. External Strategic Direction identifying health goals and program strategies that will impact on the underlying factors that influence health and health behaviours in order to achieve key health outcomes;

  3. Internal Strategic Direction, identifying key accountability and organizational goals and strategies that are necessary to achieve organizational excellence.

The purpose of this report is to provide information on the attitudes and beliefs of Leeds, Grenville & Lanark (LGL) resi-dents aged 18 and over about the ten social determinants of health (SDOH). Data was collected from 980 LGL adults using the Rapid Risk Factor Surveillance System (RRFSS).

Sexually Transmitted & Blood-Borne Infections in LGL (2006-2015)

This report summarizes the statistics and trends for new (incident) cases of specific sexually transmitted and blood borne infections (STI) in the jurisdiction of the Leeds, Grenville & Lanark District Health Unit (LGL) from 2006 to 2015.

This surveillance report on student mental health and wellbeing contains data from both primary (Gr. 7-8) and secondary (Gr. 9-12) schools situated in Leeds, Grenville and Lanark counties (LGL). The data was collected in May of 2013 from a total of 17 randomly selected schools (862 students).

This report updates the original report based on data collected in 2009. The report summarizes current mental and physical health and risk behaviour indicators and compares them to Ontario's overall student population. Where possible, trends are presented between the two time points (2009 & 2013).

A Statistical Profile of Falls Across the Life Span in Leeds, Grenville & Lanark (2003—2014)

This statistical surveillance report on falls for residents of Leeds, Grenville and Lanark Counties (LGL) was created in 2016 from our hospital reporting database (Intellihealth). The report contains data from 2003 through 2014, and is meant to identify trends in falls across all age groups, but also focuses on falls amongst seniors aged 65 years and older. The aim of the report is to assess the health status and monitor trends in morbidity and mortality relevant to falls with the goal of informing health practices and health care utilization and provide risk assessment and forecasting where possible. As well, the report is meant to support our community partners and increase public awareness about falls.

This statistical surveillance report on the perinatal and reproductive health of residents of Leeds, Grenville and Lanark counties (LGL) was created in 2014 from several different data sources. This is the first “local” perinatal and reproductive health report issued by the Leeds, Grenville & Lanark District Health Unit since the release of the Ontario Public Health Standards (OPHS) in 2008 and provides local and provincial comparisons.
