The following information MUST be provided on the new form when submitting a water sample.

  • Your daytime telephone number
  • Your name and mailing address
  • Location of water sample including Township and County
  • Date Collected
  • Health Unit Number, Leeds, Grenville & Lanark - 2243
  • Indicate how you wish to receive your results (by mail or pick up in OTTAWA)

 For telephone results peel ONE copy of the UPC Code # from the bottle, put on the blue slip provided and follow instructions on the blue slip. KEEP YOUR BLUE SLIP and call for verbal results of sample.

If this information is not complete - the laboratory has informed us your water sample will not be tested and the form will be destroyed.

The Health Unit provides a courier service for water samples. The laboratory will mail results to you if you have indicated such on the form.
