This list was compiled for families by the Association for Families of Children with a Communication Disorder (OAFCCD)

Children are to be transitioned to school based services once they turn five years of age, or are eligible for Senior kindergarten. The year that a child enters school, the Language Express Speech-Language Pathologists complete a "Transition To School Form" which informs the school that a particular child already is receiving speech-language services. This form also informs the School Board or Access Centre for Community Care that a child may be requiring continued services, thus allowing for planning of SLP needs for the next school year. Negotiations around services and transitioning protocols are planned with all stakeholders for 2001-2002 school year.

Model for Service Delivery of Speech-Language Pathology Services - To School Age Children 

Speech-Language Pathology services are not mandated through the Education Act in Ontario and the decision to provide these services has been left to the local boards. As a result, speech-language pathology services are not equally available to all students attending school in Lanark, Leeds and Grenville, Stormont, Dundas and Glengarry, and Prescott-Russell.   In order to address the needs of children leaving Preschool Speech-Language Programs and being transitioned to school-based services, a regional committee was formed to conduct a needs assessment and attempt to create a new service delivery model which would better serve our students and provide more cost-effective programs.
